MEDICAL ANIMATION TRANSCRIPT: Es posible que su mdico le recomiende realizarse una endarterectoma carotdea en caso de que sus arteras cartidas estn ms estrechas o bloqueadas a causa de la placa. En su cuello se encuentran dos vasos sanguneos importantes, llamados arterias cartidas, las cuales llevan sangre rica en oxgeno desde su corazn hasta su cerebro. Usted puede sentir el pulso de la arteria cartida en la parte lateral de su cuello. Los depsitos de grasas, llamados placas ateroesclerticas, se pueden acumular dentro de la arteria cartida. Estas placas pueden hacer que el pasaje de la arteria sea ms estrecho y tambin pueden interferir con el flujo sanguneo hacia el cerebro. Este bloqueo o estrechamiento se denomina estenosis. Si el flujo sanguneo al cerebro se bloquea de forma temporal, puede ocurrir un accidente isqumico transitorio, tambin conocido como AIT. Cuando el flujo sanguneo se obstruye por completo como en el bloqueo de una arteria cartida, puede ocurrir un accidente cerebrovascular. Antes del procedimiento, se colocar una va intravenosa. Usualmente, una endarterectoma carotdea se realiza bajo anestesia general, lo cual lo dormir durante todo el procedimiento. En este caso se insertar un tubo respiratorio por la boca hasta su trquea para ayudarlo a respirar durante la operacin. Algunas veces la endarterectoma carotdea se realiza con anestesia local. Si se utiliza anestesia local, usted se mantendr despierto pero su cuello estar adormecido. Es probable que, adems, reciba algn sedante. En el lado de su cuello junto a la arteria bloqueada, el cirujano har una incisin que puede ir desde la parte posterior de su oreja hasta un punto ms arriba de la clavcula. Su cirujano encontrar la arteria cartida y, temporalmente, detendr el flujo sanguneo a travs de la misma. Su cirujano abrir la arteria y colocar una derivacin o bypass temporal para que la sangre siga fluyendo hacia el cerebro. Luego, su cirujano retirar la placa que se encuentra dentro de la paredes arteirales. Despus de haber eliminado el bloqueo, el cirujano suturar la arteria, generalmente, colocando un parche para ensanchar la arteria ligeramente. El parche puede estar hecho con parte de alguna de sus venas o de un material artificial tal como dacrn. Finalmente, su cirujano cerrar la incisin con suturas o grapas y la cubrir con vendas. Usualmente, se deja un drenaje temporal en la herida. Despus de la operacin, se le llevar a un rea de recuperacin donde el personal controlar sus signos vitales y su condicin neurolgica.
