Removal of the laminae and spinous processes to decompress the spinal cord; B. Hemifacetotomies of L3-4 and L4-5; C. Foraminotomies to decompress spinal nerve roots; and D. Removal of herniated and degenerated intervertebral disc material at L5-S1.
